Mandatory Checklist for all Athletes*

- TAPPS Student Acknowledgement of Rules
- Participation Paperwork
If attending the physicals located on campus, please fill out and sign the SAA Physicals Waiver.
If participating in Lacrosse, please fill out the TGHSLL Code of Conduct Form.

Physical - Preparticipation Physical Examination Physicals will be offered at SAA in May for the upcoming school year. Anyone not attending SAA’s physical night should have this form filled out and stamped by a personal doctor prior to the start of the season.This year's event will be Wednesday, May 8, 2013 from 6 to 8 p.m. in the gym. Cost is $20 cash.
SAA Physicals Waiver If attending the physicals located on campus, please fill out, sign and bring to the event.
Letter Jacket Information can be picked up from Coach Mattes, Director of Athletics There are three times during the school year when varsity letter winners may order. Jacket fittings will occur at each of the fall, winter, and spring sports banquets. Students missing these fittings will be responsible for contacting the Houston Graduation Center on their own to place an order with approval from the Director of Athletics.
