All St. Agnes application materials are available below in Adobe PDF format for downloading and printing. Pop-up blockers may prevent you from opening the documents - please disable your pop-up blockers if you experience any difficulty. If you do not have Adobe Acrobat Reader, click here to download the free viewer.
To submit your application by e-mail or to type your application: Open the application form in Microsoft Word format below. Type your answers in the shaded boxes that appear on the page. When you have completed the entire application, save the document and send it as an attachment in an e-mail to ehoover@st-agnes.org with "Completed Application" as the subject line, or print the application and submit it by mail as outlined below. If submitting your application via e-mail, please mail a check or money order for the $50 application fee to the Office of Admissions at the address below. Please do not use the Microsoft Word version of the application form if you intend to handwrite your application; instead, use the PDF version of the form.
Completed applications can be submitted by mail to St. Agnes Academy, Office of Admissions, 9000 Bellaire Blvd, Houston, Texas, 77036. All applications must be accompanied by a $50 application fee.
