	Athletic Team Forms Permission Slip Mandatory for each athlete, allows athlete to practice/compete, turn in immediately upon making a team along with athletic fee. Tennis Permission Slip Transportation Form Mandatory for each athlete, turn in immediately upon making a team. Physical Print this form to take to your personal physician when you go for a complete sports physical. It is highly suggested to complete the physical prior to the start of school regardless of sport. For fall sport athletes, it is a must to have this form completed prior to tryouts.
