Application forms for the 2009-2010 school year will be available here in mid-September 2008.

To apply by mail: Open and print the application instructions and form in PDF format below. After completing the application form, mail it to St. Agnes Academy, Office of Admissions, 9000 Bellaire Blvd., Houston, Texas 77036-4683. A $50 application fee must be submitted with the application.   In addition, print the teacher recommendation forms and records release form in PDF format below and submit them as detailed in the application instructions.  All admissions materials are due by January 30, 2009.

To apply by e-mail: Open the application form in Mircrosoft Word format below. Type your answers in the shaded boxes that appear on the page. The boxes will enlarge as you type in order to accommodate your answers. When you have completed the entire application, save the document and send it as an attachment in an e-mail to ehoover@st-agnes.org with "Completed Application" as the subject line. The $50 application fee will need to be mailed separately to St. Agnes Academy, Office of Admissions, 9000 Bellaire Blvd., Houston, Texas 77036-4683 before your application can be processed.   In addition, print the teacher recommendation forms and records release form in PDF format below and submit them as detailed in the application instructions. 

If you prefer a hard copy of the application, contact the St. Agnes Academy Office of Admissions at ehoover@st-agnes.org to request an application packet be sent to you by mail.
