All St. Agnes application materials are available below in Adobe PDF format for downloading and printing.  Pop-up blockers may prevent you from opening the documets. Please disable your pop-up blockers if you experience any difficulty.  If you do not have Adobe Acrobat Reader, click here to download the free viewer.  If you prefer a hard copy of the application, contact the St. Agnes Academy Office of Admissions at ehoover@st-agnes.org to request an application packet be sent to you by mail.

To submit your application by e-mail or to type your application: Open the application form in Mircrosoft Word format below (disable your pop-up blockers if you experience difficulty in downloading the form). Type your answers in the shaded boxes that appear on the page. The boxes will enlarge as you type in order to accommodate your answers. When you have completed the entire application, save the document and send it as an attachment in an e-mail to ehoover@st-agnes.org with "Completed Application" as the subject line, or print the application and submit it by mail as outlined below.  Please do not use the Microsoft Word version of the application form if you intend to handwrite your application; instead, use the PDF version of the form.

Completed application forms should be mailed it to St. Agnes Academy, Office of Admissions, 9000 Bellaire Blvd., Houston, Texas 77036-4683. A $50 application fee must be submitted with the application, or mailed seperately if submitting the application via e-mail.  Teacher recommendations and the records release form should be submitted to the applicant's school for completion as outlined in the application instructions.  Please note: St. Agnes is no longer requiring a Principal/Counselor recommendation form.  The only required recommendations for St. Agnes are the English teacher recommedation form and math teacher recommendation form found below.
